Requirements
All applicants must have the following:
- Identification form ( Valid Florida License ; Passport ; … )
- RN/LPN/CNA Licensure or HHA certificate showing at least 75 hours of instruction. (For HHA, needs HIV/Assist with med self-administration/HIPPA certification )
- Current CPR ; BLS/ ACLS Certification.
- Level 2 Background screening (Signed attestation) and Affidavit of Compliance
- Level 2 background screening Completed.
- Two Professional References.
- Auto Insurance Card / Auto Registration.
- Proof of Residency or Citizenship.
- I9/W9 / Social Security Card.
- Payment Option ( Direct Deposit / Voided Check ).
- Professional Liability Insurance.
- Physical Exam: completed within the LAST 6 Months, stating that the applicant is free of all communicable diseases.
